	Jimmy Floyd Family Center 511 North Castle Heights Ave. Lebanon, TN 37087 Phone: (615) 453-4545 Fax: (615) 453-4543 CASUAL CARE Monday – Friday 8 a.m – 12 p.m. & 4 p.m. – 8 p.m. Saturday 9 a.m – 11 a.m. This Facility is not required by the state of Tennessee to be licensed as a childcare agency. Casual care is offered to all full members only and is not intended for use as a day care or baby-sitting service. If a child requires “ special attention” while in the facility, it is the responsibility of the parent to see his/her needs (i.e. diaper changing, feeding, medications, persistent crying, illness, misbehavior). A member of the staff will locate and inform parents when these needs arise. For ages 3 month to 12 years Limited space is available. *12-child limit Full members can leave their children for up to 2 hours a day. No child may stay more than 9 hours a week. Parents or guardians of children using casual care cannot leave the facility. We won’t allow children to be dropped off without supervision of a parent or guardian inside the facility. All participants of the casual care room are required to be accompanied through the facility by an adult. Parents are responsible for any and all actions of their children. Casual Care is for well children. Please do not bring your children if they have had any of the following symptoms: A fever within 24 hours, runny nose, vomiting, diarrhea, or excessive coughing.
